your 
child
 at

Inadvertent
 delays,

emergencies 
and 
unforeseen
 patient 
treatment 
problems, 
may 
arise 
causing
 schedule
 changes. 
Should 
your

appointment

 possible,

reschedule

Younger
 children 
and 
children 
requiring 
invasive 
procedures 
usually
perform 
better 
when 
they
 are
 well 
rested 
and
 alert; 
therefore,
 morning
appointments 
are
 recommended.

child
with 
a
 signed 
verification 
for 
the 
appointment

 to 
satisfy 
school
requirements.

 courtesy,

contact

confirm

however,

responsibility

notice

extend

Broken/missed
